North Georgia's DPT Supplemental Form
for the PTCAS Admission Process for 2013 Entering Class

• Applicants to North Georgia's Physical Therapy program must apply through PTCAS, Physical Therapist Centralized Application Service.
  
  
• Follow all instructions for submitting admission requirements to PTCAS.
  
  
• Applicants should submit the DPT supplemental form to North Georgia by August 15, 2012, for Early Decision, or by December 15, 2012.  The form may be submitted by email, fax, or mail as listed below.
